What if I colocate with another provider or practice?

You may find that your information on our online form may merge with another practice or provider that you colocate with. We apologize for any confusion around this issue. Our outreach system may group multiple doctors that are at the same location to appear as if they are in the same practice.

In order to reflect a colocation situation on our online form, you will need to visit page 4 of the form where you will see a list of all providers that we're requesting an update for. Any providers that colocate at your practice location, but do not provide care at your practice should be toggled as an "inactive" status. You will need to select the reason that says "works at different practice at this address" in order to reflect a colocation situation to us.
Then, follow your update through to completion and hit submit.
