How do I add or remove a provider?

At this time you may only remove a provider. You will need to wait for outreach from us to add additional providers. We do not know when we will reach out because that is determined by the data the health plans provide.

To remove a provider please follow these steps:

1. On the fourth page of the form click, "Yes" next to any provider(s) you would like to remove. This will change them to "No".

2. A list of reasons will appear. Select the one that is most applicable.

3. Complete the rest of the form and the health plan(s) will be notified that the provider(s) have been removed. Please note that this may take up to 90 days to update in the health plan(s)' provider directories.
