How do I add or remove a practice or location?

At this time you may only remove a practice or location. You will need to wait for outreach fro us to add additional practices or locations. We do not know when we will reach out because that is determined by the data the health plans provide.

To remove a practice or location please follow these steps:

1. Complete the first page of the form and click, "Get Started."

2. On page two click, "Active" to mark the practice or location as, "Inactive."

3. Click the check box that reads, "I confirm that this practice has closed and is no longer in operation."

4. Click, "Correct" and an option to add a date will appear (if unknown click, "I don't know closing date.")

5. Finally, click, "Submit responses to health plans."
